How to Make an Appointment
Making an appointment is easy. Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.
We know that things happen in life that you cannot control. If you have to miss your appointment, we ask that you please give us 24-hour notice. If you miss multiple appointments without giving us 24-hour notice, you will be subject to a cancellation fee.
We will work with you to maximize your insurance benefits. Because everyone’s insurance policies vary, we can only estimate your coverage in good faith, but cannot guarantee coverage due to the complexities of insurance contracts.
Your estimated patient portion must be paid at the time of your service. We will bill your insurance company for services and allow them 45 days to render payment. After 60 days, you will be responsible for the entire balance, paid-in-full. If you have questions about insurance our staff would be happy to answer them and guide you through the insurance process.
Our focus is on providing you with affordable, high-quality treatment. Because of this, we offer a variety of payment options to meet your budget. Ask us about financing options. We accept cash, credit cards, debit cards, and checks.
Monday: 8am – 1pm & 2pm – 5pm
Tuesday: 8am – 1pm & 2pm – 5pm
Wednesday: 7am – 11am & 1pm – 5pm
Thursday: 8am – 1pm & 2pm – 5pm
Friday: 8am – 1pm
Sat & Sun: Closed
